Referring Therapists
Rader Programs works in conjunction with referring therapists to
insure a smooth transition into the program. The referring therapist’s connection
with Rader Programs does not end at admission but rather begins,
as the referring therapist is integrated and becomes an intricate member of the
treatment team.
Referring therapists receive weekly updates from our multi-disciplinary treatment
staff on the progress the individual is making during treatment. Referring therapists
are encouraged to participate in person or via teleconference in the program’s weekly
clinical case conferences. During the clinical case conferences, individualized
weekly treatment plans are developed with input from all members of the treatment
team including the referring therapist. Prior to discharge, the referring therapist
is contacted to participate in discharge planning. As part of the discharge planning
process, all individuals are referred back to the referring therapist for ongoing
outpatient treatment. The treatment team is not allowed to participate in
the outpatient treatment of referred individuals. Following discharge, referring
therapists receive a written discharge summary of the individual’s progress throughout
treatment.
